CHRONIC DISEASE SELF- MANAGMENT is a small group workshop that meets for 2.5 hours weekly for 6 weeks.  Designed for people with different chronic health conditions as well as care givers.

Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out thier self-management program.

Subjects taught include:

  • Techniques to deal with problems such as frustration, fatigue, pain and isolation.
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Decision making
  • How to evaluate treatments

Participants make weekly action plans, share expierences, and help each other solve problems thayencounter in creating and carrying out their self-managment program.

Subjects taught include:

  • Techniques to deal with problems such as rustration, fatique, isolation. and poor sleep
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Pacing activity and rest
  • How to evaluate new treatments

 

 

 

DSMP

 

DIABETES SELF-MANAGEMENT is a small group workshop that meets for 2.5 hours weekly for 6 weeks. Designed for people with Type 2 diabetes as well as care givers.

Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program.

Subjects taught include:

  • Techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration.
  • Appropriat exercise for maintaining and improving strength and endurance
  • Healthy eating
  • Appropriate use of medication
  • Working more effectively with health care providers.

WORKPLACE CHRONIC DISEASE SELF-MANAGEMENT is an adaptation of the CDSMP rearranged into 1 hour segments for the workplace.  Group meets twice a week for 6 weeks.

Participants make weekly action plans , share exeriences, and help each other solve problems they encounter in creating and carrying out their self-management program.

Subjects taught include:

  • Techniques to help balance work and home life
  • thechniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Decision-making
  • How to evaluate new treatments

 

 

 

DEEP

DIABETES EMPOWERMENT EDUCATION PROGRAM is a small group workshop that meets for 2.5 hours weekly ofr 6 weeks.  Designed for people with Type 2 diabetes as well as care givers.

Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program.

Subjects taught include:

  • Understanding the human body
  • Risk factors for diabetes
  • Monitoring your body
  • Physical Activity
  • Nutrition
  • Diabetes complications, indetification and prevention
  • Medications and medical care
  • Living with Diabetes, mobilizing family and friends. 

HEALTHY HEARTS

HEALTHY HEARTS-CHF is a small group workshop that meets for 2 hours weekly for 4 weeks.  Designed to help patients with CHF as well as care givers.

 

Participants take weight and blood pressure measurements at each meeting, keep a daily record of weight, blood pressure and mood. 

 

Subjects taught include:

  • Overview of Heart Failure, definiton, monitoring and managing
  • Medication adherance
  • Low sodium diet
  • Exercise
  • Dealing with depression/stress management
